To Request an Appointment
All new patients are required to have a physician's referral, even if your insurance does not require it. This is to ensure our doctors have some background on your condition and are better prepared to help you.
We accept many of the insurance plans in the Oregon and Southwest Washington region including Cigna, Coresource, HealthNet, Lifewise, Moda, Pacific Source, Providence, Regence BlueCross/BlueShield, TriCare, United Healthcare, Medicare, and Medicaid plans.
Please check with your insurance company prior to scheduling an appointment to insure that you do not need pre authorization for your visit to be covered.

When scheduling your first consultation appointment please have the following information available so we can better assist you.
We will call you once we receive your referral letter and notes. After your appointment is scheduled, a new patient packet will be mailed to you that includes forms for registration, medical history, and review of current symptoms. These items may also be viewed and downloaded on our patient portal. A portal account can be made for you once we have you scheduled for your intial vist.
Please arrive 20 minutes before your first appointment to ensure we have all the paperwork needed is complete.
In the event you need to cancel an apppointment, please give us at least 24 hours notice so we have the opportunity to fill the vacancy. Same day and No Show appointments will be charged a $50 fee.
If you need your prescription refilled, please call your pharmacy first and ask them to send a refill request. This ensures you medications will be dispensed properly. Please allow 3-5 business days for us to reveice your request, for the physican approvals, and pharmacy to dispense.
Test results require a return to the office to see your ordering practitioner. This is in part due to the complexity of the tests we have ordered, the monitoring of your condition, and the effectiveness of your treatment. At the conclusion of these visits, your health information is relayed back to your primary care or referring provider. This appointment also gives you an opportunity to ask more questions about the results, discuss symptom changes and review treatment options. If your test results indicate that no follow-up is required, we will let you know by phone or letter. As always, if the results are critical in nature, you will be notified directly.
Family Release forms are available at the front desk,
our website, or through your portal account. If you decide to grant
family members or advocates access to your health information, complete
the form and return to the Privacy Officer at Columbia Gorge Neurology
Specialists for processing.
Disability and/or Family Medical Leave Forms carry a fee of $25.00
